Our mission is to advance the dignity of individuals with disabilities by enabling the individual to be as independent as possible. We will do this through the provision of supports and services including Supported Living Coaching, Coach, In-home support, Companion, Personal Care and Respite and Res Hab, while complying with all regulations, Core Assurances and the APD Service Delivery.
We will provide the opportunity for choice through education of activities for daily life in the community. We will stay involved in the community, advocate for our consumers and encourage self-advocacy.
Planning, Progress, and Documentation
We support each person with a clear plan, consistent documentation, and regular reviews focused on safety, independence, and outcomes.
- Person-centered goals identified from the support plan and service authorization.
- Implementation planning that outlines strategies, supports, and how progress is measured.
- Ongoing progress notes and objective data collection to track outcomes and needs.
- Regular reviews with the individual and support coordination, including health and safety follow-up when needed.
- 24/7 access to the provider for urgent concerns and emergency assistance.
